Statutory Declaration Forms

A statutory declaration is a written statement made by an individual and declared to be true in the presence of an authorised witness.

The forms for the Commonwealth and each State and Territory are different as the legislation governing statutory declarations is enacted by each State and Territory, in addition to the Commonwealth for Commonwealth and Australian Capital Territory matters.
