Affidavit Forms

An affidavit is a written statement made by an individual and declared under oath or affirmation to be true in the presence of an authorised witness.

The forms for each State and Territory are different as the legislation governing affidavits is enacted by each State and Territory. You should be careful to check that the forms listed below are relevant and appropriate for their intended use as many jurisdictions have specific requirements for different types of affidavits dependent on their intended use and the court that they are destined for.
